Message for U.S. Citizens: American Citizen Services August 25, 2023, Remote Services Event in Davao City

The U.S. Embassy’s American Citizen Services (ACS) team will visit Davao City on August 25 (exact time and location to be provided closer to the event) to provide services to U.S. citizens including:

Notary services (https://ph.usembassy.gov/services/notarials/),Acceptance of Consular Report of Birth Abroad (CRBA) applications, and

Acceptance of U.S. passport applications. Please note that passport renewals can be submitted by mail—visit https://ph.usembassy.gov/passports/ for details. While all applicants are welcome to attend, personal appearances are only required for:

Applications by minors under age 18 (both parents must also appear, if possible),

Replacement of lost or stolen passports,

First-time passport applications, or the first application after age 16, and

Applications to renew a passport issued more than 15 years ago.

The Embassy has received confirmation that the Philippines Bureau of Immigration and Philippine Retirement Authority will attend this event.

Representatives from the U.S. Department of Veterans Affairs (VA) will be present to provide the following services:

Field general inquiries on available VA benefits

Field inquiries on specific pending claims for benefits

Assist in filling out VA application forms for various benefits such as compensation, pension,

death pension, burial benefits, claim for dependents and change of address

Information dissemination and face to face discussion on PACT Act related claims processing

Personnel from the VA Outpatient Clinic will also be present to provide the following services:

Business Office Team:

· Registration/Enrollment with OPC

Scheduling of clinical appointments with OPC

Information and assistance on the Foreign Medical Program

My HealtheVet Assistance

VHIC ID Application

Nursing Team:

Provide preventive immunization (Flu, Pneumonia, Tetanus/Diphtheria/Pertussis and Zoster vaccines)

Updating vital signs, height and weight record

Assessing patient’s condition and medical need

Patient education on healthy lifestyle habits

Suicide Prevention Coordinators

Provide Suicide Prevention awareness

Veteran Crisis Line information

Distribute gift items from Suicide Prevention Program

Assessment of Mental Health appointment need

Patient Advocate

Resolving concern about any aspect of health care experience in the clinic

Social Security Administration representatives will not attend this outreach event.

On the day of the event, we will accept customers for services on a first come, first served basis. We anticipate having to turn away some late-arriving customers due to volume, so please arrive promptly and prepared with all required documents. To expedite your visit, please bring both the originals and photocopies of documents for your CRBA and passport applications. The ACS representative will keep the photocopies and return the original documents to you.

Payment for services will only be accepted in Philippine pesos. Exact change only. Service fees are as follows:

· Adult (16 and above) Passport Application (DS11): PHP 9570

· Minor (under 16) Passport Application (DS11): PHP 7830

· Passport Renewal (minor DS11): PHP 7830

· Passport Renewal (adult DS82): PHP 7540

· Consular Report of Birth Abroad: PHP 5800

· Notarial: PHP 2900
